CONSULTATIONS

Our consultations for cosmetic services are always 100% complimentary.

Consultations for medical reasons are billable to insurance, if eligible. If uninsured, or you do not have out-of-network benefits, there will be a $150.00 consultation fee that is due at the time of your appointment.

INSURANCE

Dr. Sayeed is out-of-network with most insurance carriers, however if you have out-of-network benefits, treatment may be covered if it is deemed medically necessary.

The Empire Plan - NYSHIP, Emblem Health GHI and 1199SEIU Funds are some of the plans that may cover a range of services, including Breast Reduction, Implant Removal and Rhinoplasty/Septoplasty.

We will be happy to assist you in submitting a pre-approval for services, and a claim to your insurance carrier for reimbursement.

CREDIT FINANCING

If you would like flexibility to make monthly payments for your treatment over an extended period of time, then consider one of our financing options that our practice participates with - PatientFi and CareCredit.

If eligible, PatientFi offers a 12-month 0% interest option, and is affiliated with ALLERGAN to offer you lower interest rates on your Botox & Juvederm treatment.
